How to track participant goals

Create goals, add progress updates, link funding context and prepare goal evidence for plan reviews.

Before you start

Check these items before changing participant, billing or compliance records.

  • Have the participant plan goals available.
  • Confirm whether the goal should be active, in progress, achieved or paused.
  • Use participant language where possible.

Steps

Follow this workflow.

Open the Goals tab

Open the participant profile and choose Goals. Review existing goals before creating a duplicate.

Create the goal

Add the goal title, category, description, start date, due date and current status. Use wording that can be understood by the participant and your team.

Add progress updates

Record progress ratings and notes after meaningful events, provider feedback or participant conversations. Keep updates factual and linked to outcomes.

Connect evidence from notes and reports

Use case notes, communications and provider documents to support goal progress. Avoid relying on memory when preparing plan review evidence.

Review goals before plan review

Before a plan review, check achieved, ongoing and changed goals. Create follow up tasks for missing evidence or provider reports.

Expected outcome

Goal progress is visible, reportable and ready to support NDIS plan review preparation.
