How to create case notes

Write NDIS case notes with participant context, activity type, duration, support item, attachments and billing readiness.

Before you start

Check these items before changing participant, billing or compliance records.

  • Know which participant the work relates to.
  • Have the service date, activity type, duration and notes ready.
  • Confirm whether the note should be billable before finalising.

Steps

Follow this workflow.

Start a new case note

Choose Case Notes, then New Case Note, or create a quick note from the participant profile. Select the participant first so the note is attached to the right record.

Enter service details

Set service date, activity type, duration, location, SC level and support item. If the work includes travel, add travel details separately.

Write professional content

Describe the purpose, what happened, outcomes, decisions, risks and next actions. Use factual language and avoid unsupported opinion.

Attach supporting files if needed

Add relevant documents or screenshots when they support the note. Do not attach unrelated participant information.

Save as draft or finalise

Save as draft if you need to review. Finalise only when the note is accurate because finalised notes should be amended rather than edited directly.

Expected outcome

The participant has a clear, professional case note that can support coordination, reporting and billing.
