Set service date, activity type, duration, location, SC level and support item. If the work includes travel, add travel details separately.
Describe the purpose, what happened, outcomes, decisions, risks and next actions. Use factual language and avoid unsupported opinion.
Add relevant documents or screenshots when they support the note. Do not attach unrelated participant information.
Save as draft if you need to review. Finalise only when the note is accurate because finalised notes should be amended rather than edited directly.
