What AI must not invent

• a participant quote that was not recorded
• a diagnosis or clinical conclusion
• a provider recommendation that was not supplied
• a risk level or incident outcome
• a funding decision or guarantee
• evidence simply because it would strengthen the request

A reliable drafting sequence

1. Create a dated evidence bundle from authorised participant records.
2. Separate direct evidence, participant statements and coordinator observations.
3. Ask AI for an outline with source references and missing-evidence flags.
4. Draft one section at a time rather than generating the whole report blindly.
5. Have the coordinator verify every fact and recommendation.
6. Confirm the participant's views and any provider contributions.
7. Attach the final supporting documents and submit through the approved channel.

Use reporting requirements as the quality check

The current NDIS support coordinator reporting requirements ask for participant outcomes, barriers, risks, supporting evidence and justification for recommendations. A polished AI draft is not complete unless it answers the required questions with real evidence.
