Build the form

Use the form builder to add sections, questions, required fields and instructions. Keep questions clear and short.

Preview the form

Preview the public form experience and test required fields before sending.

Send the form link

Share the generated form link with the participant, nominee, provider or stakeholder according to your consent process.

Review responses

Open the form responses area, review submissions and create participant records, documents or tasks from the information where needed.
