Voice to Case Notes for NDIS Support Coordination

Voice capture is valuable when a coordinator has just finished a call, visit or provider conversation and the details are still clear. The goal is not to keep a raw recording as the case note. The goal is to turn a brief factual capture into a reviewed record quickly.

- Open the correct participant before recording.
- State the purpose of the interaction and who was involved.
- Describe observable facts, participant choices and actions taken.
- State risks, barriers and escalation separately.
- Finish with agreed next steps, owners and dates.

- Confirm duration, activity, support item and billing status.
Use a repeatable speaking structure
A short structure reduces omissions:
- Context: "Phone call with the participant about the upcoming plan reassessment."
- Facts: what the participant said, what the provider confirmed and what documents were received.
- Action: what the coordinator did during or after the interaction.
- Risk or barrier: the specific issue and the response taken.
- Next: the task, owner and target date.
Transcription is not finalisation
Speech-to-text can mishear names, provider terms, support item numbers and dates. AI structuring can also smooth over uncertainty. Compare the draft with what you remember and any written source before finalising.
When not to record by voice
Do not record where other people can overhear participant information. Avoid capturing unnecessary sensitive detail. Follow the organisation's consent, device, retention and privacy rules, particularly if audio leaves the approved system for processing.
Keep billing outside the guesswork
Voice is useful for capturing what happened. It should not automatically determine whether time is billable, which support item applies, or whether travel can be claimed. Those choices need the coordinator's review.
What good looks like
